Hey there, I am hoping someone has input or ideas on a problem with my 2005 4.0 4x4. I replaced my clutch and rear main seal. (Kudos to Toyota that was the easiest rear main seal I've ever done.) After I got everything back together and running I keep getting 02 sensor codes. I checked the harness (failed again), cleaned sensors gently and appropriately(still failed again), then replaced o2 sensors and still get both o2 downstream sensors throwing the same code. Bank 1 sensor 2 low voltage/ Bank 2 sensor 2 High voltage. If I had no engine lights before and now this is going on any ideas? I was careful with harness and tucked it out of the way before I serviced. Any ideas?

Did you replace O2 sensor with oem ones? .aftermarket ones can be hitvor miss.could be just a quince dent that there bad . One of mine went at 90k . Maybe try disconnecting the battery for 15 minutes and let the truck re learn itself . That shit drives me insane when you work on one part of the truck and now get a problem no where you were working on

I replaced with oem sensors both sides. I have noticed on my scan tool there is indicator on the live scan that says there's issue with the evap system. I looked everything over again and cold not locate any issues. I have disconnected the battery twice for full reset and light still goes off about 20 minutes into my commute.

P0138 po157. I also checked voltage to make sure there was enough and consistent power. Almost ready to ask the stealership what's wrong and just diagnose it.

Have you verified power, ground and signal for each sensor?

I checked that they have Power and I used livedata scan to watch them function when they get to right temp. Is there still a ground concern if they have power and Cycle? Is there a seperate ground off the harness somewhere? I feel like there's something I am missing something some where. I didn't have any codes or issues in the last couple years all the way up to the day I replaced the clutch.

You'd need a wiring diagram to determine grounds and locations; given that you've got one that goes high and one that goes low, a ground issue is unlikely; they both stayed low, it might be a consideration. Have you followed all the harnesses back to make sure that there was no damage (stretching, pinching, etc) that occurred when you dropped the trans? As well, what about the cats? Are they the original ones? How any miles on them?

Well upper cats may be Original, lower cats were replaced at some point before I bought it. Truck has 211k, I bought it at 114k. I am always afraid of electrical gremlins on this newer stuff and always try and make sure all wiring is handled carefully. I actually tucked main harness that runs down the tranny out of the way on to the frame rail for removal and installation. I am unfortunately removing the transmission again. My output shaft got warn pretty good by the throwout bearing and I am adding the URD relseeve kit. I didn't know this was a thing for these Trans oh well anyways. While I am doing this I am going to carefully go through the harness and check everything to make sure nothing is damaged or missing or broken. Thanks for all the help and input from everyone.
  10. Your PCM shares the same circuit with the downstream O2 sensor as a heater relay (I believe).
I had this exact same issue when I replaced my clutch last year and chased problem forever. Had to bring it to an automotive electric guru to follow the Pinout back. It's your PCM. It costs like 3k from Toyota (just the part) then it's a dealer only install and reprogramming.
    Good luck.

Don't know if this might help, but from ToyotaNation:
Zayres, I don't know what year you have or which engine, but it sounds like it is a V6. If you notice, one of your O2s is reading high and the other low. Have you had a clutch installed lately? Or if an ATM, have you had the transmission out for any reason? If the wiring on the the 2 O2s is reversed, this is the pattern of codes you will get, and no amount of sensors will ever fix it. The sensors will plug into the other wiring connection no problem, and it is definitely possible for the wiring to be mis-routed and allow them to be plugged in wrong. This is highly unlikely if the transmission has not been out, but a real possibility if it has. To confirm, just clear the codes and then unplug one of the sensors (just 1 and only a rear one). You should immediately get a bunch of codes about that sensor only, and you can determine if the computer sees correctly which sensor is unplugged. Remember, passenger side is bank 1, driver bank 2. Let us know if this helps.

Great mother of God! I didn't even think about that! When I reinstall the transmission I will try that. I am currently doing a resleeve of the output shaft from throwout bearing. I bet that is totally the problem. I will keep you posted in the next day or day when I get it back in after work. Thank you!!!

Okay, so I got my urd resleeve kit and my new flywheel, otherone I had machined by an idiot guessing he got it to hot there was (blisters ) tiny lumps of steel raised up that were solid. Anyways hopefully I am done with dropping my transmission for a few years. I reversed the wire harness and flipped it around. I have not had a return of a check engine light. Thanks so much, that never crossed my mind. Now I am rebuilding front end with adding ome 886 to my bilstein 5100.( Fatty arb bumper and heavy winch) After this she should make it to 300k already at 211k. Thanks again for all the help.
  15. Thats awesome news! I hope it doesn't come back! Out of curiosity did your CEL pop up at startup or after driving for a while / in higher gears?

Okay, just wanted to let you guys know that after flipping the harness over for the lower sensors that was completely my issue. I had to rebuild my front end ome 886 w/5100 and new control arms cv shafts etc. Also had to relseeve my output shaft on transmission. Drove it for over 500 miles no lights. Any ways thanks again for all the help!! Now onto rebuilding my rear diff. Ring gear spit out three teeth on the freeway today randomly:/
